We offer and provide all of four levels of Hospice Care

The Centers for Medicare and Medicaid Services has defined four types, or “levels,” of hospice care. One patient may experience all four levels, perhaps in just a week or ten days of hospice services. Another patient may experience one level of care over several months of his or her hospice care. Each level of care meets specific needs, and every hospice patient is unique.

In summary we explain you what the four levels of Hospice Care are:

§  Hospice Care at Home

§  Continuous Hospice Care

§  Inpatient Hospice Care

§  Respite Care

Routine Home Care or Hospice Care at Home:

Hospice care at home is the most prevalent and commonly provided form of hospice care. It is tailored for patients who are generally stable, with well-controlled symptoms such as pain or nausea. We wholeheartedly support patients and their families in choosing hospice care at home, ensuring comfort and dignity through routine visits from our dedicated home hospice care team.

Continuous Home Care or Continuous Hospice:

In situations requiring a crisis-like level of care for the short-term management of severe patient pain or symptoms, continuous hospice care becomes essential. It is specifically designed to address acute symptoms, preventing hospitalization until routine services can resume. This specialized care is usually administered in the comfort of the patient's home.

General Inpatient Care or Inpatient Hospice Care:

For the short-term management of severe patient pain or symptoms at a crisis-like level, general inpatient care is available. This care is typically provided outside the home, within an inpatient setting like a hospital or skilled nursing facility. Our dedicated team ensures 24/7 support until the patient can return home. Our inpatient hospice care facilities combine the support of an acute-care facility with the comforting amenities of a home-like environment.

Respite Care:

Respite care offers a temporary solution, provided in a nursing home, hospice inpatient facility, or hospital. This level of care enables the primary caregiver—often a family member or friend—to take a well-deserved break. Beyond preventing caregiver burnout, respite care hospice support enhances overall caregiving efficiency. Additionally, during this care level, our team is available to support both you and your family.
