Caregiver Support Program
At Humble Hearts, we stand beside neighbors grappling with life-limiting conditions who may not yet be ready for hospice care. The approaching end of life can be an overwhelming journey for all involved, and our dedicated team is committed to assisting you and your loved ones in navigating this transition with grace, acknowledging moments of joy and celebration along the way.
Our caregiver support program offers assistance free of charge to neighbors dealing with life-limiting conditions, encompassing advanced dementia, congestive heart failure, metastatic cancer, and renal failure, among others. This service is tailored for individuals who may not be eligible or emotionally prepared for hospice care.
As part of our program, Humble Hearts deploys specially-trained volunteers on a weekly basis. These compassionate volunteers provide companionship and engage your loved one in a variety of activities, including:
ü Playing games,
ü Reading aloud,
ü Listening to music and watching a movie,
ü Looking at family photographs and sharing memories,
ü Taking a walk in the garden,
ü Spending time outdoors,
ü Watering the flowers together,
ü Playing with pets, and more.
To ensure comprehensive well-being, a nurse coordinator conducts monthly visits. During these visits, the coordinator monitors participants' health, provides hospice education, makes referrals for additional community support, and assesses participants' homes for safety. Whether "home" is a private residence, a retirement community, a nursing home, an assisted living facility, or an independent living facility, our program adapts to meet the unique needs of each individual.

What you share with us will remain private, with exceptions made only in circumstances where, for safeguarding reasons, information may need to be shared with emergency services. During our conversations, we may inquire about your first name; however, you are not obligated to share it if you prefer not to. We aim to create a confidential space where you can feel supported and understood.
